Quotes shall be based on these modifications. SPECIFICATIONS ITEMS Item No. 1: SECTION INSTRUCTION TO QUOTE CONTRACTORS: A. Replace entire two paragraph of Section Instruction to Quote Contractors Item #3 with the following paragraph which reads as follows: Quote Forms will be supplied by the Owner in this Specification. Proposals shall be made upon the forms provided thereof. No Bid Bond is required when submitting a Quote for this project.

Replace entire second paragraph of Section Instruction to Quote Contractors Item #18 with the following paragraph which changes the Substantial Completion date to July 15, 2013 and Final Completion date to August 1, 2013 which reads as follows: Construction shall commence as soon, the Insurance and Bonds have been submitted to DMPS and the weather conditions to perform the work are acceptable to produce an acceptable installation. This project shall and be Substantially Completed July 15, 2013 and Final Completion August 1, Item No. 3: SECTION : A. Replace the entire revised 3 page Quote Form attached to the end of this Addendum #1 and delete the entire Quote Form included in the specification. Note, the revised Quote Form deleted the Contractor requirement to submit a State of Iowa s Asbestos Abatement Certification Requirements with the Quote Form. The Owner shall abate any asbestos material to removed to complete this project. Use attached Quote Form from this Addendum #1 when submitting your quotes. Item No. 4: SECTION UNIT MASONRY: A. All noisy work using saw-cutting tools or pneumatic tools shall be perform before or after school classes are in section each day. Grinding of all mortar joints are acceptable during the school classes. This project shall and be Substantially Completed July 15, 2013 and Final Completion August 1, The undersigned hereby agrees to furnish labor, materials equipment and services required to perform work under the CONTRACT for the DMPS Windsor Elementary School Exterior Masonry Restoration Project in accordance with the Contract Documents, which include Bidding Requirements, Conditions of the Contract, Specifications, Drawings and Addenda for the following amount. All spaces provided on the Bid Form shall be filled in. If any space provided is not utilized by the Bidder, that space shall be filled in with the notation No Bid. The inner envelopment shall contain only the Quote Form signed and notarized. If the outer envelope does not contain the three (3) required forms signed and notarized as required then the inner envelope may not be opened as determined by the DMPS Purchasing Agent opening the quotes.

State the amount per square foot for Modular Brick (3 5/8 d. x 2 1/4 h. x 7 5/8 l.) Tuckpointing which includes grinding all the existing mortar joints to 1/2 to 5/8 depth behind face of brick and pointing the ground joints with new mortar and washing brick after the brick have been pointed. This Unit Price shall include setting and removing all required scaffolding or equipment to perform this work. Note, this Unit Price does not include any brick tuckpointing indicated on the Exterior Elevations on the drawings. This Unit Price is for the Modular Brick Tuckpointing that was not identified as needing to be tuckpointed on the Exterior Elevation. Cost per square foot ($ ) Cost Words

Provide a price per unit of brick for Replacement of Cracked or Spald Bricks per Unit State the amount per brick, to saw-cut and remove any cracked or spald brick and install new matching modular brick (3 5/8 d. x 2 1/4 h. x 7 5/8 l.) and mortar new brick in existing masonry exterior wall to match adjacent veneer. This Unit Price shall include setting and removing all required scaffolding or equipment to perform this work. Note this Unit Price does not include any saw cutting and replacement of crack brick indicated on the Exterior Elevations drawings.
