3 ABOUT THE CONFERENCE Abut the Public Health Assciatin f Australia (PHAA) Representing clse t 2000 individual members and ver 150 prfessinal rganisatins, PHAA is recgnised as the principal nn-gvernment rganisatin fr public health in Australia. PHAA wrks t prmte the health and well-being f all Australians by advcating fr better ppulatin health and equity principles. Our primary health care plicy encmpasses understanding f the scial, envirnmental, ecnmic, cultural and plitical determinants f health. This plicy, with guidance f the cnference advisry cmmittee will drive the agenda and tpics t create an interesting and engaging event. These include incrprating health prmtin, preventin, individual and cmmunity health strategies, and persncentred appraches t clinical care. Cnference Backgrund The NPHC Cnference is in it's furth year, cntinuing n frm the highly successful previus events hsted by the Australian Medicare Lcal Alliance. In 2016, the NPHC Cnference will again be prudly hsted by the Public Health Assciatin f Australia supprted by the Australian Hspital and Health Care Assciatin and many thers. Theme and Structure The NPHC Cnference 2016 theme will be supprted by cmplementary streams fcusing n Cmmunity, Clinical and Gvernance. Each stream will incrprate a strng fcus n preventin, cnsumer engagement, building capacities fr imprving patient utcmes with an emphasis n mental health, Abriginal and Trres Strait Islander health, ppulatin health, health wrkfrce, diversity in practice, chrnic disease, ehealth and aged care. Stream 1: Cmmunity Fcusing n cmmunity and cnsumer prspective, this stream will lk at ways primary health care services are aligned t lcal experiences, requirements and expectatins f the cmmunity and cnsumers. This is an pprtunity t share experiences and address issues unique t health including access t services and gaps. It will als be an pprtunity t discuss and share experiences in engaging with lcal cmmunities and cnsumers. Stream 2: Clinical Crdinatin f lcal health care systems are vital fr patients t reduce preventable hspital admissins. Primary Health Netwrks (PHNs) are required t wrk in partnership with Lcal Hspital Netwrks (LHNs) t develp lcal strategies t imprve peratin f the health care system. This stream will fcus n crdinating resurces t imprve health utcmes, pathways between hspital and general practice, streamlining patient care and cst-effective, innvative ways t imprve rural and remte access t health care fr vulnerable cnsumers/grups. Stream 3: Gvernance PHNs are required t perate effectively and efficiently t deliver against natinal and lcal gals/ targets. This stream will fcus n perfrmance, utcmes, financial, risk, legal and business management systems in primary health care. It will als fcus n the gvernance f PHNs and crdinate netwrking and resurce sharing particularly in relatin t rganisatinal structure, cmmissining, purchasing and prviding services. Overall, the NPHC Cnference 2016 will deliver a strategic, innvative prgram that has a balance f big ideas and practical examples.
