17 Precnstructin Ntes 1) General Ntes: A) Any ntes fund herein these precnstructin ntes shall be deemed an addendum t the American Public Wrks Assciatin (APWA) Manual Standard Specificatins. Highland City fllws the 2007 APWA Specificatins and Standard Plans (available at B) See Sectin IV fr all required Highland City inspectins. C) Tw (2) wrking days ntice request will be given t the public wrks secretary, either verbally (801) , written (5400 West Civic Center Drive, Suite 1, Highland, Utah 84003), r by (publicwrks@highlandcity.rg) fr all required inspectins. D) Highland City Engineering Inspectrs wrk Mnday thru Thursday (7:00 a.m. t 5:30 p.m.). Inspectins will be scheduled between 7:30 a.m. t 5:00 p.m. E) Any wrk perfrmed withut the prper inspectins taking place will be remved at the Cntractr s expense and redne with the prper inspectins. Material replacement may be required at discretin f the City Inspectr and at the expense f the Cntractr. F) All cnstructin shall be as shwn n the apprved cnstructin drawings stamped APPROVED FOR CONSTRUCTION, HIGHLAND PUBLIC WORKS with a date and signature. These plans are t be n site at all times and available t the City Inspectr fr review. If the plans are nt n site, then a stppage f wrk will be issued until the plans are available. G) Any changes t the apprved cnstructin drawings must be apprved by the City Engineer and the Design Engineer. This apprval will be given in writing by the City Engineer t the Cntractr and the Develper. Until such written authrizatin is received, there has been n apprval granted t change the plans. H) The Cntractr is respnsible t reprt any errrs fund n the APPROVED drawings, as sn as they are nticed, t the City Engineer and the Design Engineer. A written addendum, written by the City Engineer, will be issued t the Cntractr and the Develper ntifying them f the change t the plans. There will be n cst t the City fr these changes. I) The Cntractr shall take all necessary and prper precautins t prtect adjacent prperties frm any damage that may ccur frm the prject. J) The Cntractr is respnsible fr all safety requirements n the jb site and will ensure enfrcement f the necessary and gverning regulatins. 2) Utilities: A) Unless therwise nted n the plans, the existing utilities and their lcatins are taken frm research data f available recrds and nt actual field verificatins. B) Existing utilities lcated n the plans are fr the cnvenience f the Cntractr; neither the City Engineer, Design Engineer, nr the Develper will bear any respnsibility fr the accuracy f the data r fr utilities nt shwn n the plans. The Cntractr is respnsible fr field verificatin f these utilities. C) The Cntractr is respnsible t fllw all laws and regulatins regarding utility lcatins and excavating arund these utilities, including the required Rad Cut/Right f Way Permit. D) The Cntractr shall take all precautinary measures t prtect existing utilities which are t remain in place. Any damage dne by the Cntractr s peratin shall

18 be repaired and/r replaced satisfactry t the City Engineer and wning utility cmpany at the expense f the Cntractr. 3) Quality Cntrl and Testing (APWA Sectin , Quality Cntrl) A) Base i) All testing perfrmed will be at n cst t the City. ii) All cmpactin testing shall be submitted t the Public Wrks Department fr review and apprval befre mving t the next phase f cnstructin including but nt limited t asphalt, sidewalk, curb and gutter, etc iii) All testing will be perfrmed by a certified testing labratry and a certified tester. iv) Cmpactin shall meet the 95% mdified prctr requirements. v) The City Inspectr has the authrity t direct the lcatin f testing r t call fr additinal tests if deemed necessary. B) Sub Base i) All testing perfrmed will be at n cst t the City. ii) All subbase (r subgrade if n subbase is used) will have a prf rlling test with a City Inspectr n site. iii) Prf rlling tests will cnsist f driving ver the subbase (subgrade) with a fully laded fur (4) thusand galln water truck t check fr any type f deflectin in the material. iv) City Inspectr shall have the discretin t require the remval f material and the amunt at lcatins f failures. v) The failed material shall be remved and replaced at the expense f the Cntractr and retested. C) Trench Cmpactin (APWA Specificatins, Sectin and ) i) All testing perfrmed, will be at n cst t the City. ii) Testing results are required t be submitted t the Public Wrks fr review and apprval befre the next phase f cnstructin can begin. iii) Cmpactin tests are required every tw (200 ) linear feet f each utility and every tw (2 ) feet f vertical lift. iv) Maximum lift thickness is eight (8 ) inches. D) Sidewalks and Curb & Gutter (APWA Specificatins, Sectin ) i) All testing perfrmed, will be at n cst t the City. ii) Testing results are required t be submitted t the Public Wrks fr review and apprval befre the next phase f cnstructin can begin. iii) Sidewalks - cmpactin tests are required every fur (400 ) linear feet per lift. iv) Curb and gutter - cmpactin tests are required every tw (200 ) linear feet per lift. v) Maximum lift thickness is eight (8 ) inches. 4) Sanitary Sewer A) Sanitary Sewer cut sheets shall be submitted t the Public Wrks fr review and apprval by the City Engineer prir t start n sewer. B) An apprved stamped cpy shall be n site during the cnstructin. C) Timpangs Special Service District (TSSD) requires an applicatin and apprval fr cnnectin t their sewer lines applicatin submitted frm Highland City. D) Air and deflectin tests shall be perfrmed after cmplete installatin f the sewer mains and laterals and prir t installatin f any ther utility. E) Identificatin f Sewer Lines. A clearly labeled tracer lcatin tape shall be placed tw feet abve the tp f sewer lines less than r equal t 24 inch in diameter, alng its entire length [R317-3, 2.5(B)].

19 F) Deflectin test shall be perfrmed n all flexible pipes. The test shall be cnducted after the final backfill has been in place at least 30 days [R (C)(1)]. G) All sewer lines shall be flushed (flushing ball) and televised after final rad base has been installed and graded and befre the placement f asphalt. H) A cpy f the televised test shall be submitted t the Public Wrks fr review and apprval n DVD prir t placement f asphalt. I) All laterals shall be stamped with S n the curb at the lateral lcatin. J) The Sewer Lateral Recrd sheet shall be submitted t the Public Wrks befre the release f any sewer bnd mney and/r befre the final warranty perid can begin. K) Sewer Laterals shall be lcated n the prperty with a tw by fur (2 X 4) bard painted green r green PVC marker with the depth f the lateral clearly marked. 4) Drinking Water A) All laterals shall be stamped with W n the curb at the lateral lcatin. B) Main Water Lines i) The subgrade and/r subbase shall be at its final design elevatin befre the installatin f the water mains. ii) Alignment and elevatin (frm TBC) staking shall be prvided every twenty five (25 ) feet by a licensed surveyr in the State f Utah prir t the installatin f the water main. Any exceptins t this must be apprved in writing by the City Engineer r designee. iii) The ply wrap must be verlapped n all jints and fittings, and must be damage free. iv) Pipe shall be bedded and backfilled with ¾ minus gravel in the pipe zne. v) N rcks larger than three quarter (3/4 ) inch diameter shall be in cntact with the pipe. vi) FM Grease is required n all blts. C) Valves: i) All valves shall be flanged t the main, including fire hydrant branch lines. ii) Valve bxes and cvers shall be prperly aligned and fully serviceable. iii) Valves stands shall be cncrete cllared. iv) Valve lids shall be labeled WATER D) Fire Hydrants: i) All fire hydrants shall be Waterus r Mueller with a 5 bury and shall be installed s that the breakaway flange is level with the tp f curb grade. ii) A cncrete wash blck will be installed belw hydrant she. iii) Cncrete pad t be pured arund hydrant barrel belw traffic flange frm curb t sidewalk and 18-inches t each side t match sidewalk cncrete depth. E) Thrust Blcks: i) The cavity f the thrust blck shall be inspected prir t placement f cncrete. ii) The inspectr shall be n site fr the pur f the thrust blck. iii) Cncrete weigh ticket r batch ticket shall be required. F) Lading and Testing: i) Water line lading shall be dne under the direct supervisin f the Public Wrks Department. ii) All water lines will be disinfected as per State and Drinking water rules and regulatins under the directin f the Public Wrks Department. iii) All mains shall be flushed after the chlrine test and maintain a flw f 2.5 fps under the direct supervisin f the Public Wrks Department. iv) All waterlines must pass a hydrstatic pressure test f tw hundred (200 psi) punds per square inch r ne hundred twenty five (125%) percent f the wrking pressure, which ever is greater, fr a perid f tw (2) hurs. v) All waterlines must pass clifrm bacterilgical testing.

20 G) Meters and Service Taps: i) Water meter service cans will be placed in the park strips ii) Befre services are tapped, cut stakes must be installed within five (5 ) feet f final can placement. Cut stakes shall shw the tp back f curb lcatin and elevatin. Staking fr cul-de-sac and bulb/knuckle streets will require a radius pint t be staked. iii) CONTRACTOR will tap mains and set meter bxes t established grades. iv) CONTRACTOR will use ¾ inch type K cpper (distances exceeding fifty (50 ) feet in length will require ne (1 ) inch), excavatin, suitable backfill fr pipe zne, backfill, cmpactin, and surface restratin. v) Service lateral must be a minimum 10 ffset frm sewer laterals. vi) Public Wrks Department - Water Divisin will supply and install the water meters. 5) Pressurized Irrigatin A) All laterals shall be stamped with I n the curb at the lateral lcatin. B) Main Pressurized Irrigatin (PI) Lines i) The subgrade and/r subbase shall be at its final design elevatin befre the installatin f the water mains. ii) Alignment and elevatin (frm TBC) staking shall be prvided every twenty five (25 ) feet by a licensed surveyr in the State f Utah prir t the installatin f the water main. Any exceptins t this must be apprved in writing by the Water Superintendent. iii) The ply wrap must be verlapped n all fittings, and must be damage free. iv) Pipe shall be bedded and backfilled with ¾ minus gravel in the pipe zne. v) N rcks larger than three quarter (3/4 ) inch diameter shall be in cntact with the pipe. vi) FM Grease is required n all blts.
