1 ADDENDUM No. 002 For Bid Package# 500 Org. 046 Fred Florence Middle School Org. 164 Anson Jones Elementary School DALLAS INDEPENDENT SCHOOL DISTRICT September 24, 2014 The following items modify the Project specifications and drawings as follows: The following items pertain to all projects within this Bid Package: PREPROPOSAL MEETING QUESTIONS AND ANSWERS ITEM NO. 01: Both Schools Is the Asbestos report available or will a drawing be provided to identify the ACM? ANSWER: Anson Jones and Fred Florence are currently under review for asbestos. The final asbestos report and drawings will be provided by the District to the successful General Contractor following the issuing of the Notice to Proceed. ITEM NO. 02: Both Schools What is the proposed abatement schedule if required? ANSWER: Anson Jones and Fred Florence are currently under review for asbestos. The final asbestos schedule will be provided by the District to the successful General Contractor following the issuing of the Notice to Proceed ITEM NO. 03: Both Schools What is the existing roof and if under warranty, who has the warranty? ANSWER: Per the District, the existing roof for both schools are under warranty. The warranty for the existing roof will be attached to Addendum 02. Page 1 of 7

2 ITEM NO. 04: Both Schools Note on the Index Sheet indicates to install new mini-blinds at all exterior glass opening, is this applicable? ANSWER: Removal, storing and reinstalling window blinds is applicable only to the (9) nine Science Lab renovations at Florence MS only. ITEM NO. 05: Both Schools Clarification: Please revise the substantial completion date for both schools in the front end documents, sections & to be on 02/06/2016. ITEM NO. 06: Both Schools Clarification: Please revise the printing company information in section to be as following: SOUTHWESTERN BLUPRINT Attention: Sergio Gietema Address: 2108 Main St. Dallas, TX Phone: Fax: ITEM NO. 07: Both Schools Specification Sections Clarification: Please replace the plan rooms list on section with the attached revised plan room list on specification section ITEM NO. 08: Both Schools Specification Section Clarification: Please replace the MWBE form for the section with the attached revised MWBE form for section MWBE Compliance Guidelines and Forms ITEM NO. 09: Both Schools What is the next NTP Date? Answer: The revised estimated construction NTP date is projected to be 02/06/2015. ITEM NO. 10 Both Schools: Clarification: replace Specification Section with the revised Specification Page 2 of 7

3 ITEM NO. 11: Anson Jones ES A2.03 Note 1 indicates to infill wall opening. What is the extent of this work and is there a detail available? Any exterior finishes required? Answer: The extent of the infill at wall openings will be at areas where the existing radiators will be removed only. Please refer to revised sheet A2.03 dated 09/10/14, for more information. No exteriors finishes is required. ITEM NO. 12: Anson Jones ES Clarification: Classrooms scheduled to have existing wood base to be removed throughout the room will be revised remove only the wood base at the window sill only as indicated on revised sheet A0.01, A0.02, A2.03 and A2.04 dated 09/10/14. ITEM NO. 13: Anson Jones ES Clarification: Classrooms with existing wood base to be removed will be revised to remain. Contractor to prep existing wood base to receive new paint throughout as indicated on revised sheets A2.03 and A2.04 dated 09/10/14.. Color to be black. ITEM NO. 14: Anson Jones ES Is any additional structural support required for the new RTU s? Answer: Additional structural modifications may be required for AHU2 and AHU3. ITEM NO. 15: Anson Jones ES Clarification: Please refer to attached revised Mechanical and Electrical sheets E1.2B, M1.1A, M1.1B, M1.2, M2.1A, M2.1B, M3.1, MD1.1A AND MD1.1B for the following revisions: All MEP floor plans edited to correct plan designations/key plans/titles. M1.2 #1 piping roof support will work for current roof surface. M1.2- #2 revised AHU and CU designations. M3.1- #1 deleted air devices not used. M3.1- #2 CU-4 was a typo-revised wherever shown. E1.1 existing equipment pads noted on demo drawing to be removed and new pads are shown/noted on new mech. drawings. E1.2B CU-4 revised to CU-1. ITEM NO. 16: Anson Jones ES Provide specifications for the gas piping and condensate piping. Answer: Refer to Specification Section for gas piping and for condensate piping. Page 3 of 7

4 ITEM NO. 17: Anson Jones ES Provide roof type, specifications, and warranty information. Answer: Per the District, the existing roof for both schools are under warranty. The existing roof type and specifications will need to be verified with the District. ITEM NO. 18: Anson Jones ES Verify that AHU s 2 & 3 are replacing existing units. The demo sheets do not indicate to remove any units at these locations. Answer: There is currently not an existing AHU 2 & 3. AHU-2 & AHU-3 are new. The new air handling units are scheduled to be placed at existing openings that are capped. ITEM NO. 19: Anson Jones ES Provide details for the attachment of the security cage. The model number is provided in the mechanical drawings. Answer: Please refer to sheet A2.05 (dated 09/10/14) plan note 2 for further information on the security cage. ITEM NO. 20: Anson Jones ES A4.03 has a note in the upper right corner stating to reuse the new fixtures, can this be clarified? Answer: The legend is revised to only be new light fixtures not reused. ITEM NO. 21: Anson Jones ES Mechanical drawings indicate the condensate piping on the roof to be CPVC, is this correct? Answer: Yes ITEM NO. 22: Anson Jones ES A What is note number 8 indicating in rooms A109 & A110? Ceilings? Answer: Note 7 and 8 have been removed from room A109 and plan note 8 has been revised as indicated on revised sheet A0.02 dated 09/10/14. ITEM NO. 23: Anson Jones ES A0.02 indicates to remove the carpet in room A109, A2.04 does not indicate a new finish for this room. Answer: Removing carpet in room A109 is incorrect. Current floor is existing VCT and is scheduled to remain during construction. Plan Note 8 is added to only replace individual tile with new if damaged due to removal of existing radiator as indicated on revised sheet A2.04 dated 09/10/14. ITEM NO. 24: Anson Jones ES A4.02 indicates a new ceiling in rooms A109 & A110, A0.02 does not remove these ceiling. Answer: Rooms A109 and A110 are scheduled to have existing ceiling to be removed as indicated in revised sheet A0.02 dated 09/10/14. Page 4 of 7

5 ITEM NO. 25: Anson Jones ES A2.05 show an RTU that the mechanical drawings do not. Is it required?. Answer: The RTU locations have been revised to reflect the mechanical drawings as indicated on revised sheet A2.05 dated 09/10/14. ITEM NO. 26: Anson Jones ES Clarification: Environmental Contractor to remove and discard existing carpet and black mastic form Room A110. General contractor to provide and install scheduled VCT flooring as indicated on revised sheet A0.02 dated 09/10/14. ITEM NO. 27: Fred F. Florence HS It was mentioned at the pre)bid that data/communications cabling may be laying on the grid or above ceiling equipment. Is this cabling to be clean)up? If so, what is the extent of the work and what will be seen as acceptable? Answer: The existing data/communication cabling should be tied up and secured in a safe way. ITEM NO. 28: Fred F. Florence HS A2.01 indicates to infill wall openings at the following rooms A101,102,105,112,114,117)119, & 121)123. MD1.1C indicates that these rooms have window mounted A/C units to be removed. What is the extent of the repair, is there a detail available, and what is the exterior finish? Answer: The extent of the infill at wall openings will be at areas where the existing radiators will be removed only. A total of (8) A/C window units will be removed from the existing exterior window. Therefore, the windows must be repaired and a glass window will need to be inserted to close the opening. The glass will need to match the existing exterior glass thickness and type. ITEM NO. 29: Fred F. Florence HS Clarification: The DISD Abatement contractor will remove the backer rod and sealant. Contractor to provide and install new backer rod and sealant at all exterior windows scheduled to have the existing A/C window units removed. As indicated on revised sheet A2.01 dated 09/10/14. ITEM NO. 30: Fred F. Florence HS Clarification: At those rooms where existing UVs or steam radiators are to be removed, a 2 strip of existing VCT will need to be removed as part of this work. The GC will be required to install new VCT (color/selection to be selected by the Owner/AE) the width of the room x 2. The existing base will be removed and new base (to match existing) will be installed the length of the room.. The spaces that are being renovated for new Science Labs (Rooms 101, 102, 103, 104, 109, 109A, 110, 110A, 111, 114, 114A, 117, 212, 213, 320 and 320A are to receive new flooring and base as scheduled. 6 ITEM NO. 31: Fred F. Florence HS Clarification: Please refer to attached revised Mechanical and Electrical sheets E1.1B, E1.1C, E1.2A,E1.2B, E1.3A, E2.1B, E2.1C, E2.2A, E2.3A, M1.1C and M1.2A. ITEM NO. 32: Fred F. Florence HS Verify that the science casework is to be manufactured P)Lam. The past science labs that we have bid or completed for DISD have been wood science casework. ANSWER: The science lab lower and upper casework will be wood science casework with a wood veneer at faces and edges with a medium oak stain and transparent finish. Stain sample to submitted and approved by architect. Please refer to revised sheets a2.01, A2.02, A2.03, A2.04, A2.05, A2.06, and A2.07 dated 09/10/14. ITEM NO. 33: Fred F. Florence HS Clarification: Please add Plan Note 2 to Science Labs 102, 109, 111, 114, 117 and 110 as indicated on revised sheet A4.04 dated 09/10/14. ITEM NO. 34: Fred F. Florence HS Provide wall finish types at wall in)fills and repairs at radiator removal. Drawings indicate to repair, but no notes as to the wall type or finish are provided. Answer: The existing radiators are surfaced mounted and once removed the wall will need to be patched/ repaired and repainted from column to column and from floor to bottom of window sill. The existing corridor radiators are surface mounted and once removed the wall will need to be patched/ repaired and repainted 25LF on each side of the radiator locations. This includes the areas that were damaged from the radiator removal. Assume the patch will be the size of the removed radiator. The existing wall are plaster and finishes must match surrounding finishes. ITEM NO. 35: Fred F. Florence HS Clarification: Add Specification Section regarding wood case work for the Science Labs and Prep Rooms. ITEM NO. 36: Fred F. Florence HS Clarification: Add Specification Section regarding door hardware for the Science Labs and Prep Rooms. Sheet A2.01 (dated 09/10/14) has been revised to refer to the specification for hardware information. ITEM NO. 37: Fred F. Florence HS Clarification: DISD Abatement Contractor will be removing the existing millwork and disposing of the existing millwork. As indicated on revised sheets A0.01, A0.02 and A0.03 dated 09/10/14. ITEM NO. 38: Fred F. Florence HS Provide the IDF room locations for the science lab data that is to be added. Answer: Please refer to revised electrical sheets for IDF room. Page 6 of 7
