The incumbent will be respnsible fr all aspects f the prject design, develpment and implementatin. The rle will guide and supprt staff and circles f supprt t assist peple t realise their gals. The expected utcme is that every persn wh receives service frm HRT will have a PATH by end April The rle is fulltime fixed term fr 12 mnths. PRIMARY OBJECTIVES OF THE POSITION: 1. Lead and facilitate a prcess using the PATH framewrk that explres peple s wrlds and harnesses their hpes and dreams resulting in tangible and rewarding utcmes with every persn receiving services frm HRT. 2. Effectively engage a persn s circle f supprts as part f the prcess. 3. Efficiently manage all administrative aspects f Prject PATH frm inceptin t cmpletin.
2 4. Champin PATH implementatin and actively supprt peple using services, staff, family and circles f supprt fr successful utcmes frm the PATH plan. 5. Translate PATH int measurable utcmes against service specificatins. 6. Develps an evaluatin framewrk and implements. Delegatins: The psitin has interactin with all parts f the rganisatin and fllws up n deliverables, receiving inputs frm the General Manager & Supprt Lead. Financial: Staff: As agreed PATH budget Nil Nature and Scpe f Key Respnsibilities Accuntabilities PATH Develpment Lead and facilitate a prcess using the PATH framewrk that explres peple s wrlds and harnesses their hpes and dreams resulting in tangible and rewarding utcmes with every persn receiving services frm HRT Displays a cmprehensive understanding and applicatin f the PATH prcess. Actively takes the lead rle in all aspects f prject delivery. Is a dynamic and inspiring facilitatr wh is highly rganised. Uses a persn centred apprach and best practice principles in all aspects f wrk. Includes all relevant supprts in the residents life as part f this prcess. Invlved parties are excited, mtivated and engaged thrughut the prcess. Outcmes are dcumented and up t date. Supprts the achievement f persnal utcmes as identified fr each persn. Finds the right graphic artist t supprt PATH prcess. Understands and utilised all relevant legislatin including but nt limited t Privacy Act, Health Infrmatin Privacy Cde, Health and Disability Cde f Rights and the Human Rights Act, Hamiltn Residential Trust s Cde f Rights fr clients and knw where t lcate these.
3 Champins the PATH cncept in the true realisatin f successful utcmes f peples gals while using services Develps an evaluatin framewrk and implements Prvides any additinal training, resurces and/r supprt t staff, families, residents in rder t influence the PATH prcess t prduce successful utcmes. Ensures utcmes enrich the lives f residents and that they truly reflect a persn s mst aspiratinal gals. While gals need t reflect aspiratinal gals the prcess must ensure that the gals are SMART (specific, measurable, achievable, realistic and have timeframes). Reprts regularly with management and cmpletes a mnthly prgress reprt t a high standard. In cnsultatin with the trainers develps a framewrk t effectively evaluate the prject prcess and utcmes. Implements the framewrk. Reprts n prgress. Analyses trends and implements agreed changes fr cntinuus quality imprvement f prject rllut Accuntabilities Prject Management Develps a rbust cmprehensive prject plan that uses best practice prject management principles Efficiently manage all administrative aspects f Prject PATH frm inceptin t cmpletin Plan develped and signed ff by General Manager. Plan includes identified risks and mitigatin strategies. Plan includes agreed targets. Plan includes a realistic timeline. Plan identifies all resurces required. Plan includes a cmmunicatin plan. Plan includes a lessns learned lg. Plan includes the design f an evaluatin framewrk. Prject plan is reprted against mnthly with any variances identified and with a mitigatin plan. Reprts and discusses any significant issues immediately with General Manager. Maintains accurate and up-t-date recrds and dcumentatin. Ensures all dcumentatin is cncise, prfessinal
4 Ensuring PATH prcess includes effective visual requirements via an external mechanism Prject reflectin ensures learning develpment and current as per HRT plicies and relevant standards and legislatin. Makes all required infrmatin available t Users/whanau and third parties as per legislatin and plicies, stres peple s infrmatin securely and dispses f this infrmatin in a manner that meets rganisatinal and legislative standards. Cmpletes required reprting t meet rganisatinal & cntractual requirements. Utilises an efficient yet cst effective resurce fr graphical depictin in the PATH prcess. All parties invlved are satisfied with the end visual result which is f high standard. An active reflexive prcess is used t dcument key prject learnings with an end aim f cnference presentatin. Accuntabilities Reprting Reprting meets all agreed requirements Translate PATH int measurable utcmes against service specificatins A written reprt is prvided t the General Manager n a mnthly basis. Cvers prgress against prject plan deliverables. Outcmes f the PATH prcess reflect the key areas f identity, autnmy, affiliatin, safe guards, rights, health and wellness. Mnthly reprts reflect residents PATH prgress with specific attentin t the abve areas f gal attainment. Outcmes are recrded in a way that meets reprting requirements bth internal and external. Other Organisatinal Respnsibilities Quality Assurance & Risk Minimisatin Accuntabilities T maintain Cntinuus Quality Imprvement within HRT services in accrdance with the rganisatin s Quality & Risk Management Framewrk Supprt the implementatin f a Cntinuus Quality Imprvement & Risk Management prgramme within HRT in accrdance with the rganisatin s Quality Framewrk, by: Identifying imprvement pprtunities.
5 Fstering healthy relatinships with users f services, spnsrs and key stakehlders. Undertaking any training requested that supprts the rganisatin s quality gals. Participates in the delivery f the quality plan. Manages feedback prcesses. Mnitrs the effectiveness f risk management strategies and regularly reviews and reprts current risks t ensure they remain at acceptable levels. Prvides timely ntificatin t the General Manager f new risks and serius events as they arise. Participates in internal and external audits as required. Cmpletes apprpriate sectin f incident reprts and frwards t General Manager.
