Psitin Cntext THE PROGRAM Transitinal Husing Management The THM prgram is funded by the Department f Human Services n a cntract basis with Yarra Cmmunity Husing. A key aim f the THM prgram is t assist individuals and families in husing crisis t establish and r maintain apprpriate, secure and sustainable husing thrugh the prvisin f transitinal husing, infrmatin and referral services, and supprt where required. The fur cre frms f assistance prvided under the THM prgram include: Transitinal Husing prvisin f medium-term accmmdatin in which peple enter int an ccupancy agreement f a determined perid, and are managed in accrdance with the prvisins f the Residential Tenancies Act 1997 (RTA). Initial Assessment and Planning prvisin f Initial Assessment and Planning (IA&P services and, where apprpriate, husing advcacy t individuals and families in husing crisis). Husing Establishment Fund prvisin f financial assistance t husehlds in husing crisis. Public Husing Early Allcatin Assessment assessment f client eligibility fr early allcatin t public husing thrugh the Department f Human Service Segmented Waiting List, and prviding timely and effective preparatin f Segment Three applicatins. THE SERVICE Initial Assessment & Planning The IAP service prvides husing infrmatin and assessment f husing and supprt needs, assistance with accessing crisis, transitinal and lng term husing, advice and assistance with applying fr early husing thrugh the Office f Husing, referrals t husing, supprt and ther services, and advcacy. The IAP functin is ne f the mst public functins f YCH, prviding a first pint f cntact fr the rganizatin. 2. Organisatinal Relatinships The psitin f Senir Wrker - Initial Assessment & Planning Wrker has the fllwing rganisatinal relatinships: Accuntable t: Team Leader Initial Assessment & Planning Supervises: Psitin may invlve supervisin f ther staff as delegated by the IAP Team Leader Internal Liaisns: YCH Western Tenancy Teams, YCH Nrthern Tenancy Team, YCH Asset Management Team & ther YCH Staff members External Liaisns: Supprt agencies, ther cmmunity husing agencies, human services and ther relevant netwrks 2

3 3. Psitin Objectives The Initial Assessment & Planning Senir Wrker plays a key rle within the IAP team. The primary bjectives f the rle are t wrk with and prvide assistance t the IAP Team Leader with the fllwing aspects f the IAP service: Prvide supprt the IAP team in assisting individuals and families in husing crisis as a result f hmelessness r impending hmelessness t achieve psitive utcmes in their pursuit t establish and/r maintain apprpriate, secure and sustainable husing thrugh the prvisin f infrmatin and referral services, fllw up, and financial supprt where required. T ensure the effective peratin f daily IAP service prvisin and, T ensure that best practice respnses are prvided by IAP wrkers t peple seeking assistance T assist with managing the YCH IAP priritisatin and referral prcess T assist with administering the financial assistance payments system T assist with ensuring that the IAP service meets client and service delivery data cllectin requirements, and t update and maintain relevant databases and files prmptly and accurately as required. T actively participate in, and cntribute t, the rganisatin s cntinuus quality imprvement prgram fr rganisatin and service develpment, review and planning. 4. Key Accuntabilities The IAP Senir Wrker rle invlves wrking clsely with the IAP Team Leader t ensure the smth peratin f the service n a day t day basis. The Senir Wrker will undertake specific duties in accrdance with the areas f accuntability detailed belw alng with IAP duties as required. NB sme duties are shared with IAP wrkers wh may have delegated tasks in these areas. IAP Senir Wrker Duties Prvisin f supprt t IAP Team Leader and IAP Duty Wrkers t ensure effective daily IAP service delivery. Duties include: Ensuring sufficient staff cverage at all times during ffice pen hurs Prvisin f supprt and secndary cnsultatin t IAP wrkers (individually and at team level) n client issues n a daily basis Decisin making in accrdance with the YCH West Delegatin f Authrity Plicy where the IAP Team Leader is unavailable Prvisin f supprt t IAP wrkers delegated t assist with maintaining the YCH IAP priritisatin and referrals fr IAP clients requiring case management supprt and/r supprted transitinal husing Prviding secndary cnsultatin n IAP service prvisin t external agencies Wrking with the IAP Intake wrker t ensure external agency c-lcatins wrk smthly thrugh mnitring agency attendance, appintments, IAP referrals, crdinating appintment rm availability etc Ensuring service prvisin areas are well maintained at all times t ensure a welcming, clean and safe envirnment. This includes prvisin f infrmatin resurces in the frm f brchures, well maintained nticebards, maintaining the DHS infrmatin kisk. Direct IAP service prvisin as required (refer t IAP Psitin Descriptin fr details): The senir wrker prvides assistance t the IAP duty service as needed including respnding t phne inquiries and IAP duty wrk as needed. Wrking with the IAP team Leader t ensure timely reslutin f service prvisin and team/wrker related issues. Efficient payment f Husing Establishment Funds t accmmdatin prviders. Duties include: 3

4 Maintaining HEF payments system Undertaking all required IAP administrative tasks and Liaising with the Office Manager t ensure timely payments. Prvisin f IAP utreach respnse activities as required including c-rdinating IAP respnse, in cnjunctin with allcated IAP wrkers, t rming huse r ther lw-cst accmmdatin clsures in YCH IAP catchment. Maintenance f a psitive and cnstructive wrk envirnment t ensure cnsistent, best practice IAP service prvisin t clients by mdelling apprpriate behaviur fr IAP wrkers including timeliness, respectful and prfessinal ffice cmmunicatin, prfessinalism, accuracy f file ntes and practive prblem slving. Service Develpment: Participate in nging achievement f YCH Accreditatin thrugh DHS One Standards.
